To assist eBrightHealth Accountable Care Organization (ACO) participating practices in performance improvement and transformation activities to achieve annual quality and operational goals as established by the ACO Quality Improvement Committee.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Conduct monthly review of practice performance reporting Support practice and health system quality improvement initiatives Communicate ACO-specific initiatives and assist workflow optimization development Supports practice education of Hierarchical Condition Categories (HCC) optimization Supports execution ACO Action Plan and Key Actions Participate and support node- and practice-level engagement efforts to achieve established quality, utilization and cost operational goals
ADDITIONAL RESPONSIBILITIES INCLUDE:
Assists with annual ACO regulatory reporting requirements for clinical quality measurement at both the individual practice and health system level.
Perform regular practice reviews to determine compliance status with ACO participation and performance requirements, while providing identification of available resources to assist as available.
Provides oversight to and support for Participants in the Focused Improvement Plan process so that they can contribute positively to the ACO's clinical, quality and financial goals.
Identify opportunities for improvement and alternate strategies when goals are not met.
Assist in the identification of EMR enhancements needed for practice improvement and transformation initiatives.
Facilitate and motivate personnel and leadership from throughout the practice and affiliated health system and ACO to implement evidence-based practices.
Performs assignments that need to be completed, even when outside the scope of routine responsibilities; adaptable to changing departmental conditions.
SCOPE, PURPOSE, AND FREQUENCY OF CONTACTS:
Works collaboratively with physician and operations leadership, CareVio care management leadership and front-line care coordinators.
Frequent contact with departmental personnel, Informatics, Data Analysts, Medical-Dental Staff, Medical Records Staff, hospital personnel, physicians/staff that are ACO participants and executive leadership for the ACO
Intermittent contact with external personnel as assigned for the development and ongoing operation of Registry and EMR development.
Reports directly to the ACO Quality Officer.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor's degree in relevant discipline required.
Minimum of five years of related experience with two years in patient safety, quality or performance improvement or practice management.
Knowledge of medical terminology and anatomy required.
Knowledge of or experience in any of the following is preferred: computers, data abstraction and entry, outcome analysis, utilization management, clinical experiences in patient care processes, medical records, performance improvement, and team facilitation and education (patient/staff) processes.
Organizational skills and project management required.
5 years' related experience with 2 years' ambulatory patient safety, quality or performance improvement may be considered in lieu of a Bachelor's degree.
