This position is located within the Chronic Pain Wellness Clinic of the Nursing - Ambulatory Care Services, The Phoenix VA Healthcare System (PVAHCS) in Phoenix, AZ.is a Joint Commission Accredited, complexity level 1a tertiary care facility serving Veterans in Phoenix, Arizona. PVAHCS is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology and research. The Chronic Pain Wellness Clinic registered nurse (RN) demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The RN demonstrates proficiency in decision-making and using the nursing process for complex situations that effect performance outcomes at the point of care. Practice is enhanced through development of collegial relationships. This nurse demonstrates collaboration by using the group process to solve problems. The focus is on leadership in strategically planning long-term goals for the microsystem (unit, team, or workgroup) that impacts operations or resources. The proficient nurse demonstrates critical thinking with efficiency and accuracy in assessing and managing situations. This RN is responsible for collaborating with services internal and external to the VA to facilitate coordination and transitions of care to effectively meet the patients' needs to include interventional pain care coordination with the outside facilities. The RN is accountable for assessing, monitoring, supporting, educating, and effectively coordinating the care with mental health & chronic pain care providers within system. The RN duties will include face to face and phone visits for highly complex patients with comorbid mental health disorders such as addiction, PTSD, depression, anxiety, etc. and chronic pain. The nurse completes Clinical Reminders and Suicide Risk Assessments, triage, substance use screening , de-escalation, and therapeutic interventions for high risk patients, as appropriate. The RN assists with chronic pain related procedures.
