Utilizes data and provider/staff referral to identify patients most at risk for complication or admission to the hospital. Follows these patients closely in chronic care management developing a care plan, providing education, monitoring and communication to assist these patients in self management and wellness. Stays up-to-date on community resources and monitors quality of those resources for our patients. Works closely with hospital care management team to provide warm handoffs from hospital discharge to ambulatory clinic follow-up, assuring that transition of care appointments are made in a timely fashion. Utilizes data to close care gaps, i.e. mammography, colon screening, well child visits, immunizations etc. Active member of the quality improvement team assisting with monitoring of data from various sources and always looking for opportunities for improvement.
