Peachford Hospital is seeking an experiencedÂ Outpatient Therapist.
Located in the Dunwoody neighborhood of Atlanta, Peachford features individual units for children, adolescents, adults and seniors, serving residents of Georgia and beyond. We offer inpatient acute care, as well as partial hospitalization and intensive outpatient programs. (www.peachford.com)
The Outpatient Therapist will provide quality direct and indirect patient care to individuals with substance abuse or chemical dependency problems.
Key Responsibilities of the Outpatient TherapistÂ include:
Collect and analyze data regarding the patientâ€™s physical, psychosocial, environmental, self-care, educational, and anticipated discharge planning needs.
Demonstrate knowledge and ability to assess and provide appropriate interventions that are unique to the needs of the patient population (age and program specific) being served.
Coordinate treatment team meeting daily for assigned physician team.
Assist physician to coordinate patient plan of care, which includes rounding daily on patients, documenting findings and gathering pertinent clinical and UR information.
Communicate pertinent treatment and discharge plans to other team members.
Communicate changes in patientâ€™s status (legal, psychiatric, physical) with attending physician.
Participate in facility QI process by auditing records/systems as indicated and communicating results and opportunities for improvement.
Support facility internal and external customer service standards.Â
